Supporting documentation needed to apply for a British passport - Child (By birth in the UK).
The following documents must be presented in support of your child’s passport application. Original documents are always required and will be returned on completion. Photocopies will not be accepted.
Each item on the following list of documents is annotated as ESSENTIAL or DESIRABLE to your child’s application. If you do not have all the ESSENTIAL documents, your child’s application cannot be accepted. Your child will need to be present on the appointed day of the application assessment.
1. BIRTH CERTIFICATE (one of the following is Essential)
· Child’s Long Registrar Office BC (contemporaneous) or
· Child’s Long Registrar Office BC (recent replacement) or
· Short Registrar Office BC (contemporaneous) or
· Short Registrar Office BC (recent replacement)
2. IDENTITY DOCUMENTS
· The passport that your child travelled from the
· The passport that your and/or your spouse used to travel to the
to your child’s birth and the passports you returned to
· Current identification for both of you as parents (Essential)
NB: If your child is claiming British Citizenship through you and you were born, adopted, registered or naturalised in the
· Evidence of how you became British: e.g. original birth certificate or original
naturalisation certificate (Essential)
· If you are not British and your child was born after 1/1/83, evidence of the
status of you or your spouse in the
· Child’s ID Card or Nigerian Passport (Essential unless too young)
3. LIFE PHOTOGRAPHS
Please have these organised (labelled with names and dates and in order) before attending
· Photographs of your child as a baby with you as parents in the
you and your child can clearly be identified (Desirable)
· Photographs of your child on return to
siblings (if any siblings) (Desirable)
· Photographs of your child growing up in
siblings (if any siblings) (Desirable)
· If you, the parent, have married you also need to submit your wedding photographs. (Desirable)
4. TIME OF BIRTH DOCUMENTS
These should be one year before and up to two years after your child’s birth
· Mother’s original antenatal/postnatal/delivery notes from hospital. If unable to obtain
original records, please submit copies, which have been certified by hospital. (Desirable)
· Health Cards, from the hospital, doctors or clinic (Desirable)
· Immunisation/vaccination cards (Desirable)
· Receipts for the birth, medical treatment (Desirable)
· Baptism certificate, Family Allowance book, Baby Book (Desirable)
· Letter from Hospital confirming birth (Desirable)
5. PARENT’S DOCUMENTS
· UK Marriage Certificate if married in the
· NGA Marriage Certificate if married in
· If either parent has married before, Decree Absolute of prior statutory marriages (Essential)
· Photographs from wedding ceremony (Desirable)
· College Diploma / Certificates (Desirable)
· Employment / tax records, salary slips (Desirable)
· Building Society Books/ Bank statements (Desirable)
· Medical Cards (Desirable)
· Driving Licence / ID Cards (Desirable)
6. CHILD’s SCHOOL DOCUMENTS
· Reports, Results & testimonials for all schools attended and all years (Desirable)
· School Certificates for all exams (Desirable)
7. DEATH CERTIFICATES
· If father, mother or any sibling is deceased a DEATH certificate (not medical
cause of death certificate) must be submitted (Essential prior to issue )
8. NAME CHANGES
· If you or your child are now using a different name to the one on your birth certificate then be prepared to present a sworn affidavit (stating which name you wish to be known as) and documents showing what names you have been using. (Essential prior to issue)
· If the name change is through marriage you will need to present your
marriage certificate and photographs from your wedding. You will also need
to provide identification for your husband. (Essential prior to issue)
· If names are abbreviated in your child’s documents please present Sworn
Affidavit to support this. (Essential prior to issue)
